Description
Pretty decent. However...
Adding in date of birth field (my own plus child) - the pop out calendar aligns right - ss attached. Difficult to work. Similar if you choose yes for are you pregnant.
DOB fields, once info is in, the number is hidden (year element) ss attached
NO footer (there should be please!)

Also, Newton has nixed the calendar on these fields so are the calendar overlap issues now moot?
