Description
Create a new page with a form. Similar structure to contact us - can be simple enough.
COntent of page & form attached.
Access
From the menu Submissions-> APpeal a Decision.

This is nearly done except I'm having problem with their requirements for the file upload.
So Newton will finish out:

- Status changed from New to Resolved
- Assignee changed from Newton Duarte to David Bryan
- Status changed from Resolved to Closed
